Would you transcribe this with hypens or not. 

Doctor diagnosed below-average-to-average intellectual functioning...

I would only hyphenate below-average. - NM

hyphenate below-average, above-average, etc., but I would not hyphenate the whole phrase.

There is no need whatsoever to hyphenate above or below average.
