Awoken vs. woken vs. awakened - haylee

I am struggling with this sentence a bit and seem to find conflicting articles about how to properly do it. Doc dictates, "When the patient was awoke, he was unable to verbally communicate." I know 'awoke' is not correct here, but I am struggling whether it should be 'woken' or 'awakened' or 'awoken'.... not even sure if 'awoken' is a word. LOL. Help Plz!

I would use awakened - you probably already sent report, though
