They don't know it is a problem.
Stop blaming the doctors. They don't do the contracting, they don't have any instructions, and they hate what your company returns to them as much as you hate doing it.

That there is no communication with them about it. The hospital I was at for 10 years would talk to the docs face to face to ask them to stop belching, yawning, chewing (LOUDLY), plus to get their paperwork and thoughts organized prior to dictating so as not to waste our time. We even let them listen to themselves dictating and hear how it sounded. I think they just have too many more important things to worry about and just forget. I don't think they're doing it to be rude.... they're just distracted with their patient's health and well being.
