If you work for a MTSO, you usually are not allowed to communicate with the docs. If he/she is your client, then you could. That said, the OP's first inclination to use numerals is correct--1st, 2nd, 3rd, etc., to promote clarity in the health record. They are usually written out in literature and correspondence--first, second, etc. I did an extensive amount of podiatry during my career and used 1st, 2nd, 3rd, etc., always; but it would be the way to do it in any type of medical report, not just podiatry.
