Absolutely agree, done it that way for years. Unless site specifics state otherwise, change "pee" to "urinate," etc. This is a medical record. It could well end up in a courtroom and is not just for insurance reimbursement, as some on this board have said. Even if the record stays dormant in cyberspace forever, it has my name on it and "doo-doo" is not how I roll.
