I'm not 100% clear on what numbers you need, but your last pay stub from December 2019 may have the hours worked as mine does. If not, you will need to figure how many hours you worked. If you work 40 hours per week, divide your total pay by 2080 to get your hourly rate for the past year. If you're applying for state medical benefits, they will probably use last year's information on your W2 to get those numbers (mine did). Hope this helps a bit.
