It is entirely possible that a doctor's worst nightmare would be a mindless but perfect "scribe" that would document their every uttering precisely as produced (as if that were even possible). Even scarier: What if a general atmosphere of inaccurate documentation might be seen as an advantage (as in: "prove it"). If there's an upside, it is the incentive for every single patient to closely monitor the accuracy of their own medical records.
