My doctor told me recently that when he pulls a patient's chart from the hospital EMR to review now, there is a disclaimer on the bottom that basically states that the accuracy of the medical record cannot be verified because it was transcribed by speech recognition software, etc. That's just terrible and so disheartening, but not surprising, as we all know...
