department of a large hospital, one of the doctors dictated the instruction to "type a negative", which meant to use the template for whatever test the patient had. Well, the poor guy's legs were full of clots and the doctor's instructions were incorrect. Of course, the first person they looked at was me and I heard from the transcription supervisor at the hospital that the doctor wanted me fired, that it was a serious error on my part, what was I thinking, blah, blah, blah, blah. The audio file was reviewed and sure enough, she was wrong. Did I get an apology????
