First, you should have been notified you were on a PM within the past 5 weeks. Like previous poster said, they are supposed to fall off after you have met the requirement for 6 weeks after the PM. If not, only then should you get escalated to a second level.
There are so many reasons for not meeting LPH. Maybe consider working in a focused way for a few power hours at a time, taking a short mental health/stretch break, then again working for a few hours, and not allowing yourself to get side-tracked by emails, internet stuff, or anything else.
If you eliminate all your distractions and make an effort at speeding up, and you still have poor LPH, it could be you are getting too many crappy reports, which is not something any of us have control over and slows us all down. If that is the case, tell her you will try to do the Magic Incantation to Bring on Good Dictators, No ADT Issues, and Clear Audio. What else are you supposed to do??
That is why having a minimum LPH is a pet peeve of mine. If we are meeting our quality requirements and not costing them make-up pay, what do they care how many lines we do? WE are the ones losing pay over it!
