I did advise the patient that we would give her Cephadyn for relief of her tension headache symptoms, one every four to six hours as needed for severe headaches, maximum of three per day, dispense #40, and s/l PRIME 11 5/300 one every six to eight hours as needed for headache pain, #75 x0 refills. Patient was also given a prescription for Zofran 4 mg one every six hours as needed for nausea and vomiting as well as Demerol 50 mg one every four to six hours as needed for severe migraine headache, #40 x0 refills. Did advise this patient that this will need to last her for nearly two months, as she is pregnant. Patient states that she has been on s/c PRIME 11 in the past for her severe migraine headaches. However, I am not certain that I will be prescribing this again in the future

In indications, the patient has a very long history of sinus issues and vertigo.  "She was complaining of headaches on both sides over the maxillary sinus.  She was convinced that her sinus trouble made her more *pretiginent**bretigent*"  Something along those lines.  I am not even sure what letter it starts with!
