Why would MME change - Frustrated

mom to mother on a VERBATIM account at every instance of dictator clearly saying mom? Is there something I am missing? I wish they would leave some kind of note stating why this was changed. Client profile clearly states "this is a verbatim accouint" and "type as dictated." QTL does not answer email. I usually ignore these things, but sometimes it is hard not to get frustrated when you leave 1 blank and get back several corrections for this kind of stuff. Thanx for the vent.

We're almost to the point where we have to leave a QA note that the account is verbatim, and not to mess with anything.

Mom to Mother - Old-school MT

Years ago, we were told that mom and dad were sort of slangish (like kids) and to change to mother or father (and children). MME is either from the old school, or just wanting to be more formal, or he or she got corrected as an MT by an old-schooler or something.

Just let it go. You are almost always safest just typing as dictated unless it's off the wall. Let this be on the MME end on her audit. Notes below about chilling out. Your supervisor doesn't have time to answer a question like "why did she do that?" for something like mom vs mother. Save your e-mails for something really, really big and you're more likely to get a response. I have had tons of supervisors and have always gotten a response, because I don't clog them up with little things like that. Don't give them a reason to want to ignore you! Remember the boy who cried wolf? Nobody heard him anymore...

I believe in order to do our job properly, we have to have a clear understanding of what is expected of us, and so does the MME. If this is a verbatim account, it should NOT have been changed.
I have 1st hand experience of getting a job back from QA that was no longer accessible, and I got the write up as well as the MME because there was not proof of who changed it.
I keep a note pad of all the job numbers sent to QA and how many of the blanks they filled in compared to how many I left for this exact reason, that way I know who changed what.
I also let my QTL know of stuff that is changed incorrectly because my head is not going on the chopping block for anyone anymore!
This has nothing to do with crying wolf... those reports can only be access for a certain time and the report I got back was 2 months old, not accessible.
If the QTL wants to ignore me for asking for clarification, then so be it... then if I get back another report, I will have proof that the QTL did not do her job in fixing it.

If I have to be accountable for my job, then anyone who touches it better be accountable for what they do.

I have gotten Customer Care Bulletins and blanket emails from CCM strongly reminding all MTs on certain accounts to type what the dictator says, and these are not strictly verbatim accounts. We are told NOT to change mom, dad, poop, pee, etcetera. This is not considered slang, but the dicator's style.

Drug terminology and some procedures would be the only instance I would change something that is consider slang.

I would bring this to the CCM's attention. It is not crying wolf, whatever that means. We have had docs specifically bring this up as something that bothers them. They have reiterated that they just want it typed as the dictate it.

I did not think it should go to the CCM but I did say it absolutely should go to the QA Team Lead!
The team lead is the one who performs audits and who lets the MMEs know when they are wrong to change this stuff.

I sort of got off the point with the crying wolf thing, but it sounded like the OP just wanted to do what she knew was right, and expected the CCM to get into the MMEs head. I was trying to explain what might be in the MMEs head, but that it doesn't matter. The OP clearly knew that she (the OP) was in the right and I'm recommending that she carry on, and let those things be on the MMEs audit.

I was trying to explain that if we clog up the CCM/QTLs e-mails with these things (when we know what we're doing is right), the big stuff gets ignored.

MMEs. I have had so many changes made to my reports that are INCORRECT that it's almost unbelievable. MQ has the lousiest QA I've ever experienced anywhere.

Oh, and note that I did say SOME. No need to flame me for stating the truth.

She said that our decisions should follow first the CP, then the MQ Guidelines, and if nothing else helps the BOS. The BOS is not the Bible - it changes constantly.

She also said that when we change things, for instance mom to mother, many clients feel that we are just padding our line count - doesn't matter if it's right or wrong.

Verbatim accounts are just that - we type what is said and nothing else. If the doc says something really off such as 5000 mg of morphine, we leave a marker and a "question the dosage" note. Basically on a verbatim account all stupidity falls back to the doctor who spit it out.

post all the crazy things they find and fix. I know of several MTs who have been saved from grave consequences by MMEs especially upon correcting serious ADT mistakes and copies mistakes; but let's not forget about the really horrible mistakes in diagnoses and lab work that they fix daily. It is shameful how folks here go on about such minor silly things. Everyone knows there is another side to the story. Folks who complain so much are usually not very good MTs.

considering the MTs are audited with regularity and such minutiae are counted towards accuracy, it is very important to have standardization of terms so as to avoid confusion and poor scores on our audits.

The MME should be absolutely positive the changes being made are appropriate and CORRECT. An employee in that position is expected to have a greater degree of accuracy and knowledge (theoretically anyway).

All complainers should become MMEs. - It is ridiculous to use

the MMEs and then complain about changes that are not incorrect. Become an MME. MMEs do not have to send reports to QA. They get to correct the reports of others and find out the real overall poor quality of the "types" who complain about such things. Then the complainers can be complained about.

Of course we know that most of them have already tried and failed and this is the reason they complain.

Likewise, the MTs need to be sure the changes - are inappropriate and/or incorrect

Hello, My $0.02, I understand that you are speaking of a verbatim account. However, many, many times I have changed nonverbatim accounts mom to mother and QA changes it back to mom. I always notify QA that per BOS 2 (the version we are to go by) that on pg 169 under "family relationship names" it clearly says to edit to the formal term.

I can't tell you how many times I've been burned on that one. I figure if we are to follow the BOS 2, then EVERYONE should follow it as well. It shouldn't be subjective.

We had management changes over the weekend for my new account and the new manager sent out e-mails today that we have to get new e-mails, must start using some probably crap butt IM program (I HATE IM!) and they insist that if your account runs out of work that you wait 10 minutes, then e-mail them then wait up to 15 minutes to see if they can find you some work before you can move to your secondary if that account has work. I'm now transcribing directly into the EMR for my client now, and therefore, cannot charge by the lines anymore. The only thing I can think of to do is charge by the megabyte. I would put the whole billing period of voice files in one folder, click properties, and there would be my total megabytes for the billing period. I played 8 voice files that added up to 27.81 minutes and 2.95 mg of voice files so it seems safe to assume 1 mg = 1 min of dictation. At $1.85 per me ...
