QA question....has this happened to you? - So Tired

As stated in my previous post, I have been typing all new accounts for the last 2 weeks with no training whatsoever. I got a "critical error" report back because of the number of blanks I had put into the report and sent directly to the client. When reviewing the report, I noticed that some of the blanks were not caused by me at all, but by the QA person rewording the dictation to his/her liking. Furthermore, this QA person had re-added in prohibited abbreviations where I had taken them out (added back in q.h.s. and deleted "at bedtime" which I had typed). The CP clearly stated not to use prohibited abbreviations and to type them out. Why is ithat the QA person's submission is the final word even though it is obviously incorrect?

I was just reviewing the report and the "critical error" was an error made by the QA person!!! She stated that I had incorrectly put "docusate" in the patient's discharge medications, but after nit-picking with all of the other corrections, didn't change "docusate" names 3 MORE TIMES IN THE DICTATION as the patient had constipation....so tired of idiots!!!

This is a no-win situation. My primary has vanished, and I'm in the cesspool too. Many of the accounts only have a minimal CP - only instructing the format for ROS and PE.

We get punished for leaving too many blanks, but I wonder if QA/MME gets penalized for not getting our blanks

be left as is. I know, I know, they change it as often as the wind blows. We are only to follow the JCAHO prohibited abbreviations, which is a very short list, unless, of course, the client profile says otherwise.

I swear, the only reason they make and then change the MT rules is so they can sell their book every year to the same people. In the next few years, it will be back on the list.

h.s. and q.h.s. are listed on page 209 of BOS III as additional recommended selections for dangerous abbreviations, and on page 212 of BOS III as part of an ISMP dangerous abbreviations list.

Are we supposed to follow the BOS which does have it, or the JCAHO list which doesn't have it?

I get soooo confused with all of these changes all the time!

the only way she would have known that the doc said q.h.s. and not at bedtime is if she listened to it. At our company the only reason QA listens to the whole file is if you are on 100% QA as a new hire.

...or if the dictator or facility or work type is on full QA. Having done QA for many, many years, sometimes we "see" things then fix them. Not sure what's up with q.h.s. I did not know that was on the JCAHO list of dangerous abbreviations. Not sure what AHDI says these days. In any event, I wouldn't get too crazy over it. Unless it's truly on that JCAHO list, I doubt on an audit it would be low points off, if any at all.

I just to recently switched to a whole new region, supervisor and QA supervisor with 3 days notice because of ONE particular account I transcribe on. I had been typing the previous accounts for over 5 years. Occasionally when I'm doing an ASR report and insert a QA marker the yellow cursor marker will go away and the cursor goes to the end of the report. Then I have to scroll back through the report to find my place and if I lift my foot off the foot pedal it happens again. Does anyone know how to fix this? It's irritating on a really long report. That is when I got a new CSM and they said they were "restructuring" and some CSMs were going to deal only with MTs and some only with customers. My new CSM said her old MTs were always running out of work because THEY said the average MT could do "x amount" of dictation and most of hers could do twice that, and so when she got the "new-to-her" MTs and accounts, then we all started running out of work. ...

I am reading on this board and seeing that people from Medquist are being let go, places are being bought out, hospitals are selling in-house to the outside services now.
