More to remember - this is ridiculous

Dangerous abbreviations that are in BOS are not to be observed unless the specific CP says to? I have NEVER operated like this, EVER. So now we get to check the CP and try and remember the BOS fix for the dangerous abbreviations (used to just keep it as a shortcut). I gotta get out of here, my mind doesn't process all this crap.

Yes, that was my problem too. I learned after working an account for years that OS, OD, and OU weren't in dangerous abbreviations anymore, so we were to type as dictated. Sometimes, in assessment, I wasn't too clear on this, so would constantly have to ask because of conflicting e-mails. Then there is the issue of client profile. If it is not listed in client profile where you were NOT supposed to type this dangerous abbreviation, you would type it. On other things not listed in each client profile, I would get points off in audit if I got it wrong. So before doing each report, I would have to read the client profile. I tried reading each client profile in my SPARE time, but with each work type the client profile changes, so no way to prepare.

We are supposed to memorize the company standards and guidelines, and then the client profiles only list the exceptions to the S&G. My head is going to explode. I didn't know about OS and OU from the above poster. Geez, I guess I have been doing it all wrong. Wonder what else I don't know. I can't keep walking through this minefield for minimum wage.

If the doctor says it, I type it. I don't give a flying duck what BOS or the CP says. I don't have time for that. When they start paying me hourly, I'll start doing things their way. As long as they only pay me for the black characters I can produce, that is all they are getting from me. They can fire me if they want. So far they haven't.

That's what I've decided to do regarding the cc/mL debacle, unless something is in the dumb CP. I consider everything qualified verbatim basically...lol!

I would like to hear more about this freelancing. I am not asking for the name of your company or anything personal, but how did you go about getting into freelancing? Do you make any money at it? What type of work do you transcribe. How much training did you need? Is there plenty of work to do? Anything else you would like to share would be appreciated, as I am quite interested.

I have thought about applying to the elementary schools as a substitute teacher, which seems to be in demand. The pay is not great, but you work when and if you want, and you get off holidays, weekends, spring break, Christmas break and the summer. All of that time off is without pay of course, so you would really have to do some financial planning. That is what my husband did after he retired, and they ended up hiring him full-time, so the supplement to SS income is nice.

They also have an open prn position at the hospital in my town, kinda like a substitute, filling in for people who are out. The university here has the same type of position. I was also thinking about applying to both. I could lead to full-time, but I would have to go on Obama care until it did. That would be more work in the summer because that is when everyone wants to take vacation but, again, you work when and if you want. However, I like the sound of the freelance transcription too. MM has beat me down so bad. I am still here, but have picked myself up again, and look forward to when my job with them actually ends!!!

I fell into my current freelance gig by accident. A casual acquaintance and I got to talking one day about our professions. She is a photographer, I was an MT. She was simultaneously working on 3 or 4 projects involving life histories and photos that accompanied those histories, two of which are going to be made into books.

After I'd explained to her what medical transcription was (because she'd never heard of it before, like most people), she asked if I'd ever transcribed interviews before. Turns out I had, and she asked if I wanted to do some work on the side of MT for cash. There was no specific deadline, so I said sure!

I've been doing freelance work for this one person for about 2 years, now - a little at a time. She's been very patient about waiting for it, since I had to fit it into dry-spells with MT. Recently things just fell into place for us: She had 2 huge new projects, one genealogy project and one book, that needed transcription from interview recordings. At precisely the time she offered this work, I got laid off from my MT job. So, I'm doing the freelance work for her almost full time now.

It doesn't fully support me yet, and I'm still drawing unemployment as well. But as a friend, I'm also giving her a deal: I'm charging $40/hr. for the transcription part for easy work, $60/hr. for difficult ones with accents, background noise, or lots of research time. Then, when it's ready to go into book form, I'll be learning some new software for working with the actual page setup of the books, and insertion of photos. That will be charged for separately from the transcription.

Once the unemployment runs out, if the sideline isn't fully supporting me, I'll probably go ahead and start drawing my social security.

Anyway, what this all boiled down to for me was just being in the right place at the right time. I never bothered to join Elance because the bids were coming in globally, from (you guess it!) India, Pakistan, etc., so I have no intention of getting into bidding wars with anyone. If I need more work, I'll be more likely to find what I want through word-of-mouth and referrals, than most of the junk-jobs that are found online. So yes, luck played a rather large role in the current work I'm doing, but I'm sure everyone else out there can find gigs in this manner, as well, and not just typing jobs, but any manner of things. Word-of-mouth is by far the best advertising, whether you're in the market for a job, or in the market for an employee!

It is all very confusing. I was not familiar with the dangerous abbreviation list in the BOS, but the facility where I work as its own list of acceptable abbreviations, including a "dangerous list." I think some of these may come from JCAH. I always thought CP came before the BOS.

Ok, what confuses me is that there are a lot of abbreviations on the TSG 'acceptable abbreviations' list (TSG Appendix B) that are on the BOS dangerous abbreviations list (BOS Appendix B).

For example, q.h.s. is a TSG 'acceptable abbreviation' but then qhs is on the BOS dangerous abbreviations list.

But, I type q.h.s. (unless CP says otherwise), because the order of authority is: CP, TSG, BOS.

Until they pay me for my time, I will not waste it looking things up. If the doctor says it, that's how it goes into the report. If they expect me to know more than the doctor then they need to pony up the doctor money.

If they don't like the way I do things they are free to fire me. At this point I would welcome it.

The order of our transcription rules goes like this:

1) Client Profile (CP)
2) Transcription Style Guide (TSG)
3) Book of Style (BOS)

CP rules trump all other rules.
TSG rules trump BOS rules.
Then BOS rules if not covered in above rules.

Alles klar? LOL!

I am in coding school (home-based program) and as I read through the coding book, I try to understand all of the rules, etc., about what gets coded first and dual codes, when to use a particular code as a primary diagnosis, etc.  However, I find that I remember little of those rules the next day or even later in the day.  How do you remember all of these things, or do you even have to?  Is everything I am reading about rules also included in the instructions in the tabular index?& ...

You actually looked forward to getting the PA instead of the doctor because they were so much easier to hear, understand, etc., except for the fact that most of them have no clue when it comes to pronouncing medications and feel that it is above them to spell the medication out.  In my case it seems that the roles have flipped nowadays, I absolutely hate seeing the PA coming up and would rather do the doctor any day of the week!!     ...

I sent 1 to QA with 1 blank in it so does it go by that or the number of things they decided were wrong with it, because this person totally picked it apart changed things such as operation done at xxx hosptial to done in xxx hospital, etcetera.  It is not a verbatim account either.  I don't recognize the QA name either.   ...
