Don't doctors and PA-Cs know no abbreviations in diagnoses? - MT

Why the constant slang and abbreviations in diagnoses and impressions?  Especially when it could mean more than one thing ( for example I and D) and it cannot be gleaned from the record.  Are these clinicians lazy or what. I think there should be stricter guidelines for those dictating, rather than placing the call on the MT.  I know I am preaching to the choir so to speak, but it is really getting on my nerves...

I remember way back when, when I worked inhouse, and we had a medical student come and apply for an MT job to help him pay his bills and get through school. Not only did he not expand abbreviations under diagnoses and impressions, he abbreviated things the dictators actually did not abbreviate. We had to explain to him that we have rules, and that not everyone who reads these reports is a doctor.

If they say "I and D" and you don't know which they're referring to, you leave "I and D" regardless of where it is in the report. CYOA

They might not know that because no one told them. Or, they might believe it is your job, because that's whose job it has been until now.

I'm having a problem with this part of the diagnosis as she doesn't specify to go on to the next number, as she does with the rest of them, but I've never had someone that adds the procedures used to fix the diagnoses in this section.  This is the sentence as she says it, "1. I am doing a DS.  I can't believe this has never come up before (or I've forgotten!) - When typing LAVH-BSO on the diagnoses, can I use LAVH-BSO or do I need to spell it all out?  I would think LAVH-BSO would be more readable, but I know the rule is to spell it out.  I'm trying to be practical here, but it may come back to haunt me. Just wondering if anyone knows where I could get a list of the "dangerous abbreviations" in transcription.  I was at a doctor's office one day and saw a sign on their wall that said something about dangerous abbreviations and something about things that transcriptionists should not type...but I left thinking that it would be something easy to find but have found no information on that at all.  Thanks so much for your help! ...

I don't use dangerous abbreviations and I never abbreviate in the diagnosis and QA has been changing my reports to abbreviations in the diagnosis and changing words and adding the dangerous abbreviations. Does any else do this, not abbreviate etc? ...
