Help!  What is "inspected" when micronodular cirrhosis of the liver is noted?  It sounds like:  "Farapacodrene" was inspected and the micronodular cirrhosis of the liver was noted.

could it be "fatty change?" nm - ?

I doesn't sound like fatty change. What precedes the statement is: Upon entering the abdomen cavaity, a moderate amount of ascites was identified and evacuated. THEN the doctor states (in a foreign accent so hard to understand) what sounds like 'FARAPACODRENE' was inspected and the mircronodular cirrhosis of the liver was noted. I've checked everything and can find nothing for 'farapa-codrene'

If you are allowed to make any minor edits, abdominAL would be more correct in the adjectival form, and it would be cavIty. Ascites is excess fluid in the space between the tissues lining the abdomen and abdominal organs. I assume this a laparoscopic procedure?

You are all so sweet to help....no, it's not portal hypertension. It definately sounds like 'Farapa-codrene'...or 'The Rapacodrene'. Gosh, it's so hard to understand.

I think it should be "portal hypertension" rather than photo hypertension. Could the word you are hearing be "retroperitoneum?"

Don’t know what the procedure is, but I found this operative report that seems to be along the same lines.  Maybe reading this over and then comparing terminology with what the dictator is saying will help you figure out what they are saying.  Sorry I could not be of more help.  


Nitrous oxide is most commonly used to insufflate the peritoneal cavity.  The Veress needle is removed and a disposable trochar inserted through the skin incision.  A 5 mm oblique viewing video laparoscope provides excellent quality views of the abdominal cavity.  Inspection of the liver should follow a routine.  Firstly, the falciform ligament is inspected for portal hypertension (the presence of dilated veins), followed by the left and right lobes of liver, gallbladder and peritoneum.  The inferior and posterior segments may be visualized using a probe, inserted through a separate site, to displace the liver.  Liver biopsies may be taken.  In patients with ascites, fluid may drain for 24-48 hours and should be collected in a colostomy bag.

This is in an Op Note for a rectum excision. "The fatty tissue was dissected off the sigmoid circumferentially so that there would be s/l cirrhosis, as well as approximation without a layer of fat between the s/l cirrhosis." Both words clearly sound like cirrhosis but I cannot find a reference to anything that makes sense since this has nothing to do with the liver.  Help if you can please. ...
