Wow, now I have something that could have caused someone to die. Right at the end of a long report, VR shows the patient is DNR DNI, when he really said the patient is full code! How easy that could have been missed! ...

I have a great part-time IC job that I really love. It is so stress-free and I only hear from my account manager when she sends me my pay summary right before pay day. What a joke. They come down on us so hard about this, but there is information on ADT we have no reason to see such as SS numbers. I have a couple of dictators who actually dictate the patient's SS number and say to put it on the report. Not to mention the wrong patient information and cc names given by the dictators.
