If there is no other clue in the report as to which one the dictator is saying, how can you determine which to use? Are they pronounced exactly the same? TIA

pronunciations be? I am aware of the definitions of both, and this is in the past medical history, so it could be either. As I said above, there are no other clues as to which it may be. So I am wondering what the difference in pronunciation is.

Without some kind of clue, you really have no way of knowing for sure which it is. For instance, some neurologic disease can cause swallowing problems. On the other hand, if a gastroenterologist says he is being consulted because of DISFAZHAH, you can guess with a more than reasonable degree of certainty that it is dysphagia.
Because of this, some savvy doctors will pronounce it DYSPHAZHA, but in reality they are pronounced alike, so it's up to the transcription to know.
Swallowing and speaking - anon

Dysphagia is trouble swallowing - it has a "g" like gag.
Dysphasia is problems with speech production - has an "s" like speech.
You have to listen to the text and try to figure out what problem the patient has - speaking or swallowing.
Am aware of definitions, that's why I said no clues given - anonym
