There is no comma usage here - it is 20/50-1, or with a space 20/50 -1. Depending on your account/physician specifics, you do not need the decimal points either (-1.00, just -1), but if you are required to do so, then obviously keep them in.

Need correct format for this - Dr. dictates "I did angioplasty and stenting to his mid RCA with a 3.5 x 23 vision and placed 2.5 x 12 vision to his first diagonal and his EF was about 35%, a 50%. He dictates it "3523 vision" and "2512 vision". TIA! ...

He says"NOP vision" (says N, O, P each letter separately) a few times throughout report. Here is one sentence:
D: It was felt that most likely s/l NOP vision was from optic neuropathy/optic ischemia from high intraocular pressure.
