For normal exams they often use templates of standard text that can be inserted into the transcription so they don't have to dictate it. I'm not sure what platform you are using or how where you would find this. You might just put a note on it saying (template) and refer it to QA who can tell you where to find these templates.

but all before he dictates "template" dictates a whole paragraph of all other ranges of motion, and usually he will say "within normal limits" if nothing is unusual. I will ask QA.
Okay, I'm stuck. This doc is dictating his physical exam and is talking about the cervical spine rom. "Full range of motion of the cervical spine to flexion, extension, rotation, side bending, and (s/l) discompression sign."
Does anyone have any ideas? ...
on pe, He has normal subtalar and s/l "mu talsa" range of motion, however, he lacks dorsiflexion with 0 degrees bilaterally. He needs to stretch.

Cerebellar testing is somewhat abnormal in that she has a sort of a s/l stirtuous motion that finger-to-nose is hesitated but she does get finger-to-nose testing.

Does this sentence make sense to you or am I just getting it wrong? Seeing as how you're not supposed to move around during MRIs, this makes no sense to me. Or maybe I need a lot more coffee today and/or a day off... ...
PE on neurologist consult. Pt with spinal sarcoma. Doc is a pretty clear dictator. Not sure if "tesseril" is actually "tests for all" or what. How about "effort". Is this customary in a neuro consult?

I don't think I've had this before. I've had trisomy 16 and trisomy 22 but she clearly says more than once what sounds like either partial trisomy "sixteen to twenty-two" or "sixteen, two twenty-two." If it's a range, is it normally transcribed as 16 to 22 or 16-22? Thanks in advance. ...
