How should this sentence be written? 

He had 2 out of 20 lymph nodes with (metastasis or metastases) and extranodal extension.

Also,

The patient was found to have pathology hepatic (metastasis or metastases) on follow up study.

Metasis (is is singular), metastases (es is plural, more than one).

Second maybe "The patient was found to have pathologic metastasis on followup (noun) study.
