Itching vs scratching - shred

If a doctor dictates itching instead of scratching, as in patient was itching the area, do you change it to scratching? Or is that too much editing?
Would depend on the account instructions - how verbatim do they want it

There is one dictator on my primary account who had to be made strictly verbatim because he threw a huge hissy fit whenever anybody changed his word in the slightest... "when I say the patient will have to wear bags on the belly, I MEAN 'bags on the belly!"
So in your case, I'd check the AI, and if that's of no help, ask your supervisor what they think, and get it in writing via e-mail in case it comes back to bite you later. :)
Doc dictates "The patient has rather intense (pharitis?) in the calf area that I saw him scratching."
Could possibly be "varitis" with a "v" and not a "ph." This is a pretty clear recording and a doctor without an accent, but I am stumped. Any thoughts? ...
