Hemoccult is it capitalized...and Hemoccult-negative ??with a hyphen???

There are 2 (plus or + ) dorsalis pedis pulses.

Hemoccult is a brand. And based on gram-negative, I would also say hemoccult-negative would not be capitalized, though no one may complain if it is.

The Gram stain in microbiology is used to classify bacteria. But when you talk about gram-negative or gram-positive cocci or whatever I don't think it is capitalized. That is using it as an adverb-adjective noun modifier, so a hyphen. And Gram is a proper noun like Hemoccult and not capitalized when used as an adverb-adjective combo.

Use the + sign. No space.

Do you have a Book of Style? AHDI, formerly AAMT, has changed some of the rules for their third version - I can only imagine to try and stay relevant and in the $.

Is the brand name of a fecal occult blood test, so yes, capitalize. As far as the hyphen, if nothing comes after Hemoccult negative, no hyphen; but if it is Hemoccult-negative test, results, etc., yes, put a hyphen. Do you have a style guide you could refer to, like BOS 2 or BOS 3? The answers to most all your questions can be found there.
