The National Institute of Health, Office of Rare Diseases has it listed as tako-tsubo at this website rarediseases.info.nih/gov. I consider the NIH to be the most credible source. Making it one word would be done if it was a doctor's name. Tako tsubo means octopus traps in Japanese.

I had no idea about the derivation of this name, it's so interesting. Thanks, and I agree that tako-tsubo is correct.
I do see that takotsubo is widely used, as well. (www.ncbi.nlm.nih.gov/pubmed)
