CHA2DS2 VASc - Super

We have a physician that dictates this "His chads vasc 2 score is 2.2% a year."  Our system cannot take superscript, would you recommend we "correct" this to the correct term, i.e. CHA2DS2 VASc?  I cannot find anything on web searches for CHADS VASC2 scores.

I would type it the way you have it - CHA2DS2 VASc. Will see what others say, but I think that would be the proper way to type it.

Even though it goes against what I would prefer to transcribe, I think I would give the MD what they dictated: CHADS VASc 2 (or maybe VASc2) score. I'll be interested to see what others think.

Thanks everybody for your responses. I think I will go with CHADS VASc2 since we cannot really be expected to remember to add the 2s in the midst of the acronym.

It seems like every doctor dictates this differently (CHADS VASc2 score, CHADS 2 score).  I'm having trouble finding the exact guidelines for this. For example, if the doctor dictates it as CHADS VASc2, should we type the "official" term as CHA2DS2-VASc?    ...
