Blister on toe is s/l un/hearthed ?? see msg - an

A small incision in the blister is made with a #11 scalpel.  There is some serous fluid but no pustule material.  This in PE.

In IMP/PLAN she states::

As noted, the blister is unhearthed ?/ and  a swab of the internal contents are sent for culture.  Any thoughts or help is appreciated.  Thanks!

sounds more feasible than unearthed which is a term I have never heard in transcription.

the doctor says "There was no skin to pull back as it had been accordioned together into an (s/l) ischemic mass" I am at a loss here! Help please.  TIA ...
