could it be 0.5 mg 2 b.i.d. to equal the q.i.d - 4 times a day? Simply does not make sense otherwise.

seems to be going from dosage, 0.5 mg, given four times daily to twice daily - gee! Not a good or clear way to document, tho, it seems to me.

They probably meant to say 0.5 four times a day or 2 twice a day.
With it being a medication - ask for clarification

Don't guess or take anyone else's suggestion on a medication dosage, especially with something like Ativan.
Type it exactly as dictated (which makes no sense) and flag it for clarification by the doctor or office.
ah-tee-vin. Now if we can get all the doctors to change how they say ativan, we're set...because apparently programming it correctly in ASR is too difficult. ...
