Hello there! When they're referring to a nadir, they are referencing the lowest point of, for example, a particular blood test or other measurements. The word nadir is used often in my acute care situation, but, again, in your sentence above, "His creatinine has nadir'd at 119/3L," would mean the 113/3L is the patient's nadir, the lowest point of the creatinine quotient the patient has had thus far.
