potential client, promising perfection all the time, and over the years all responsibility for making a medical record readable falls on our shoulders. The doctor holds no culpability, the hospital is not in charge of reminding the doctors, PA's, NP's, and residents that they have to make their dictation style perfect, their on-payroll legal team isn't reminding the hospital to remind the doctor to proofread the reports to avoid malpractice, etc. If the medical record fails in any way, it's our fault.
This is why we veterans are leaving the field, because we refuse to be the whipping boys of this profession.
