Here is how I would type it:

Left upper extremity: His arm to his abdominal wall proximally is sutured in place and distally at his wrist area, but his hand is free where it exits his abdomen; this is not sutured in place.

The sentence does make sense for that type of abdominal flap procedure. All it needed was correct punctuation placement. Hope this helps :)
