I was wondering if any of you have come across when physician's dictate or write DSM5 diagnoses and they capitalize the first letter of each word. Do you transcribe it the same way or should they be transcribed as regular, ie Major Depressive Disoder, would you type as shown or major depressive disorder. The diagnoses are being listed in paragraph format, and I could not find anything in Book of Style that references something like this... Any input would be appreciated.
