That was totally uncalled for. At least she's asking.
To OP: Punctuation always goes only inside the quotes only, with the exception of a question mark in certain instances. Examples below.
The patient asked, "Should I take the medication only once a day?"
How should we handle the patient's remark "I don't want to follow the treatment plan"?
In my first example, the patient is asking the question, so his question gets the question mark inside his quotes. In my second example, the doctor is asking the question.
