I would not recast or reword anything. You are almost always safer typing as dicatated, unless it's completely unclear to the reader, or if it compromises patient safety. If your report gets audited, rewording or recasting would be up for grabs in terms of a ding.
Doc speak is allowed in reports, unless you are otherwise instructed.
