I don't know about those factors, but the Rule of 6 would be a deal breaker

The rule of 6 does apply to the ROS. Basically, whenever the doctor dictates the ROS, whether he breaks it down by systems or just runs statements together, you stop typing once you reach 6 items, even if it breaks up systems awkwardly and no matter how it is dictated. This means you are literally stopping in every ROS on EVERY. SINGLE. REPORT to COUNT out the words. It HAS to be solely for the purpose of slowing down the productivity, because there can be no other reason. For example, dictated:

HEENT: No runny nose. No sneezing. No sore throat. No cough. No ear pain. No eye discharge. No teeth pain. Chest: No chest pain. No chest tightness. No wheezing. Abdomen: No abdominal pain. No heartburn. No vomiting. No diarrhea. Extremities: No extremity edema.

Typed: No runny nose, sneezing, sore throat, cough, ear pain, or eye discharge. No teeth pain, chest pain, chest tightness, wheezing, abdominal pain, or heartburn. No diarrhea, vomiting, or extremity edema.

You literally have to reword the sentences from negative and positive (usually how it is dictated but PF doesn't allow you to type) to has and no and then just list the items and then count them until there are 6, which the doctor usually dictates as a group by system, so that completely throws you off your rhythm. If someone can explain any logical reason for this, I am all ears.
