if you keep adding things that aren't there, the speech engine will never, ever learn (and it has a hard time as it is, lol). They want these documents fast and cheap. It's another mindset, I know, but just listen and make sure what's said is what's there. If your account specs do not say to add "the" before patient, then don't. Just make sure major errors are corrected and/or flagged, even if speaker states it wrong (i.e., says 120 mg when drug is only dispensed in mcg). HTH!
