I just looked at mine, but it looks like it is saying that the $123.08 is the lowest cost to the employee if they take the insurance. Funny thing is, I've NEVER been offered any insurance through them. I believe that if any of the other boxes are not filled out (covered individuals, etc), then it was not taken out of your check as insurance. I could be wrong, but that is the way I am reading it.
