I'll say...no fan of EMR and EHR here, at my job we basically insert an old-style dictation job in between the endless repetitive automatically generated lines of the EHR form. It looks like utter garbage. You really cannot tell who typed what or when, and of course errors get repeated over and over again because nobody knows how to change the base template.
I know who thought this would be a good idea -- a person who knew absolutely SQUAT about medical records but was going to go ahead and try to reorganize the entire system anyway, and likely for a huge financial reward.
