How does the engine learn correct punctuation and grammar if the dictator isn't telling it where to punctuate?

How does it learn to type "2" instead of "two"?

If the engine only learns via voice, then why does it always type "2015" instead of "2018" when dictated.

I've always been told the transcriptionist plays a vital role in training of voice rec.

I don't think anyone knows how this works.
