I might consider not hyphenating unless you're using it as an adjective. But definitely TAD (transcribe as dictated). If you have a responsive QA team, you might consider asking about it for later reference. For example, when CAT scan is dictated, we are instructed to type CT scan. So even on "verbatim" accounts often you're required to do some editing.
