It is per choice, or if someone you type for requests a specific format, but is grammatically correct to say:
per Anesthesia protocol (which is what I type)

or per anesthesia protocol. (because for some reason a lot of capitalization has been dropped in transcription just as they no longer capitalize the seasons: Summer, Winter, Fall - but I still capitalize Fall to differentiate from the word fall as if someone falls.

Dr. Brown's protocol is correct because it is a specific name and anesthesia protocol is not possessive because it is not a person. You would have to type "per the Anesthesiologist's protocol, Anesthesiologists' protocol or anesthesiologist's/anesthesiologists' protocol
