If the doctor dictates it as number 11 then put a #11 blade was used.
if the doctor does not say the word "number" then put an 11 blade was used - it goes by what is dictated.
As for hyphens, the new electronic medical records do not handle hyphens very accurately and I have stopped using them whenever possible so that they are not misinterpreted as "negative" or - (minus).
