I have very rarely seen a QA person respond to any of the posts. Makes me think they are just what is posted about them.
I too find it all so beyond the limits of WTH. We are not writing a novel, we are documenting. I get dinged for commas, doesn't count account me, but are you serious, commas!
Again we have to be 100% and yet, the ones who are totally in charge of the patient, namely the physician/PA/NP/client don't seem to stress about it.
