Granted, occasionally I will get an Indian doctor who is clear and well spoken, though I have no idea where they grew up or went to school. For all I know, these are second generation docs here in the U.S., and I agree some may be the brahmin-caste transplants you describe. But 8 times out of 10 the Indian doctors I get, as good as they may be at medicine, don't speak English well at all, and that's why I think they are a good example of how higher education does not necessarily make one a good speaker of a second language.
