I used to work on an M*Modal knock-off program and it liked two spaces after a period and sometimes it didn't. That drove me nuts. The one versus two spaces is set up in the VR according to the client's wishes. It is built into the program, just like headers.
I am not sure I could follow your dilemma about the assistant surgeon, which probably means it is worse to work with that problem in person.
The industry and AHDI wants us all to be typing uniformly. Then why can't account specifications be exactly the same for all of the clients across the country? While they're at it, have a limit as to how many pages we have to memorize (three sounds good to me).
