There may be several answers but this is the way I would do it:
"There are times when my disease makes me feel lonely. Would you say, strongly agree, moderately agree, somewhat agree, disagree, or never?"
I don't see the need for so many quotation marks since the interviewer is asking a question and even the question is a comment made by the interviewer, the entire comment is going to be in quotes. Just my opinion.
