Quotation Question - EKitchens

I'm working on a medical interview segment where the interviewer asks the respondent to give him a direct answer. I'm not sure about my quotation mark placement.

The interviewer says, for example:

There are times when my disease makes me feel lonely. Would you say, "Strongly agree, moderately agree, somewhat agree, disagree, or never"?

There are times when my disease makes me feel lonely. Would you say, "Strongly agree," "moderately agree," "somewhat agree," "disagree," or "never"?

There are times when my disease makes me feel lonely. Would you say, "Strongly agree, moderately agree, somewhat agree, disagree," or "Never"?

And now that I'm looking at it again, would "There are times when my disease makes me feel lonely" need to be in quotation marks also since the interviewer is reading it?
