including that position with all the other positions that have long been described as "physician extenders," positions like PA, NP, etc.

Look at some medical practice websites. A significant number of them will very likely list physicians and "physician extenders" or similar description. Usually, these are paraprofessionals, but there is nothing that says the office cannot consider other positions as an "extender" of services.

There is nothing sinister or outrageous about it at all. They can call the position whatever they want. There is no law mandating medical office job titles.
