For goodness sake, do you know how confusing and stressful that is to the rest of us who are not the ones responsible for making the mistakes?
Send it to the ones doing it, and tell THEM exactly what is wrong. Don't make us all guess whether it's us responsible and what did we do or not do. That's even worse and do you have any idea how much time it wastes?
No, it's not picking on or discriminating against anyone. It's a little something called TAKING RESPONSIBILITY.
