Vera Pyle. T refers to tumor extension, N refers to nodal involvement, and M refers to metastasis. An example of the transcription would be T1, N0, M0 indicating no extension or node involvement.

Googling TNM staging will only be as good or reliable as the source you access. Many of the sources you find there give inaccurate information when it comes to transcription style because the authors of the source information either don't really know or don't really care about whether or not the word is capitalized or in all caps or followed by spaces or whatever. The late Vera Pyle was an icon in our industry for decades. She was one of the founders of AAMT back before that organization went to the dogs and the original founders washed their hands of it out of disgust. Current Medical Terminology has always been a valuable reference tool in my library. She took the time to make sure the information was as accurate as could be documented at the time, and this reference shows the TNM classification with commas and punctuation separating each part of the nomenclature.

AHDI is into making money. One of the ways they do that is by selling stuff. IMO, they come out with new editions of the BOS in order to generate new revenues, but in order to create a new edition of the BOS they have to change something or it will just be the same book reprinted. That's why you see style points that were once valid and acceptable now no longer considered as such and being replaced by changes in style, which changes are backed by often times bogus reasoning at best. Most of these changes are unnecessary and are designed to justify a new book in order to generate new revenue. The BOS is like most everything else AHDI gets it hands on - it's getting worse and was never worth what they charged for it in the first place - again, IMO.

May I remind you that, as seasoned transcriptionists, we take into consideration the fact that doctors are the last people we would want to consult regarding the accuracy of transcription style or punctuation or report formatting or wording or pronunciation or anything else one can think of that has anything to do with the healthcare documentation process. Doctors don't know what they're talking about when it comes to healthcare documentation and they don't want to know because they don't care. To them, good enough is good enough. They don't understand the ramifications that kind of attitude has for patient safety, and they don't want to understand. Let us not forget that fact when referring to doctors and employment in their offices and the preferences in style they demanded then and continue to demand be adhered to.
