intake and output - way to write - Ang

Can someone tell me if what I put below is the way to write intake and output, i.e. if I am placing the spaces properly:
This is what it s/l was dictated:
"Intake output was 14 96 X 20 100."

i.e. 14 96 x 20 100 - saying it like that, as though there 4 numbers.

[ In Reply To ..]Is he saying "by" or "times?" I doubt he's saying times. I type this 1496/2100 when they say "by". I/O 1496/2100. He may just be talking slow. This means in 1496, out 2100. Also, unless he is specifically saying I 0, I type I and O, then 1496 and 2100.

Does anyone know if there is a new rule in MQ land about never writing out the date, always making it numbers only? I get back reports every once in a while with all the written dates crossed out and replaced with numbers yet the CP does not specify either way. The patient's peritoneal carcinomatosis index is 34. This is as follows: Region 03, region 13, region 23, region 33, region 42, region 53, region 63, region 73, region 82, region 92, region and 102, region 112, region 123.
